It is mandatory to submit the PSLF form each year, or when you switch your job. This allows you to qualify for this program. MOHELA will then transfer the loan. Once you have completed the application form and submitted it, you must include any documents that prove you are still employed.

To get the PSLF job certification it is required that you give personal information. This comprises the name of your employer, Social Security Number, date of birth and address. If you want to confirm any changes made you’ll be asked to tick a box. After you have completed the form you’ll need to tick a box to acknowledge that the information has been accurate. Then, you must complete and sign this PSLF employment certificate form. This will be sent by the Department of Education.

You can recertify employment status by filling out the PSLF form, if you work for a qualifying public agency or service. Be aware that the form could influence your PSLF payments.
