Limited PSLF Waiver Form – A PSLF is used to report to Social Security Administration, or IRS the amount of payments you pay to cover the costs of your pension as well as other benefits. PSLF Payments

Before January 1st 2013 only federal student loans from the Federal Family Education Loan Program could be considered for PSLF.
PSLF can be used for Direct Parent PLUS Loans, as long as the parent holds an eligible job. Parents are only eligible for PSLF if their Direct Parent Plus Loans are converted into Federal Direct Consolidation Loans.
Recertification of employment
It is crucial to certify employment on the PSLF application form to be eligible to participate in the PSLF loan cancellation program. In order for the PSLF loan forgiven program to be valid applicants must have worked for an organization that is public in nature, such as a government agency or non-profit organization for at most 10 years. They must also make 120 eligible loan payments during the period of time. The applicant must fill out the PSLF form to confirm your employment. MOHELA will then review the form and inform the borrower the amount of qualifying loan payments they’ve made.
Each year, or whenever your work is changing You must fill out the PSLF Form. This will help you qualify for the program. The form will also transfer your loan to MOHELA (the agency that runs it). Once you have completed the form and submitted it, you must attach any supporting documents to prove that you are still employed.
To get the PSLF job certification, you are required to provide personal information. It includes the name of your employer, Social Security Number, date of birth and address. The form also asks you to check the box to confirm any changes to the information. When you’ve completed the form you’ll be asked to confirm the authenticity of the information by checking a box. Then, you need to complete and date the PSLF Employment certification form, that will be delivered to the Department of Education.
If you’re employed in a government agency that meets the requirements or public service, the PSLF can allow you to recertify your status as an employee. Remember that this form can affect your PSLF payments as well as late partial payments as well as lump sum payment.
